Care Manager (Children’s Services) works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to children and adolescents as well as caregivers in a child-centered, family focused approach utilizing evidenced-based practices delivered to fidelity. These services are delivered in multiple types of settings including field-based services in the child’s home, school or other identified locations as well as clinic-based services provided from multiple Children’s Behavioral Health (CBH) clinic locations in San Antonio. To provide intensive case management services to families of children with severe emotional disturbances (SED) by engaging the family and establishing a strengths-based relationship to identify, create, and build a supportive Child and Family Team for the purpose of developing a Plan of Care that is family centered, community oriented, strengths-based, and highly individualized.


The Youth Empowerment Services Waiver, also known as the YES Waiver, provides comprehensive home and community-based mental health services to children and youth between the ages of 3 through 18 who are at-risk of institutionalization or out-of-home placement due to a serious emotional disturbance (SED).  


  1. Provides direct clinical services, completes clinical assessments, and develops Wraparound Plans for YES Waiver participants.
  2. Links and refers consumers to appropriate services as identified in the Wraparound Plan.
  3. Ensures clinical documentation is submitted in accordance with Center standards, and meets all designated requirements.(i.e. assessments recovery planning, provide routine Case Management, Crisis Services) 
  4. Coordinates and facilitates ongoing child and family team meetings on a monthly basis.
  5. Continuously reassesses the needs of the child and family with the team and modifies the Plan of Care as needed.
  6. Monitors the provision and quality of services provided to the family through the team and is the liaison when new services/resources need to be sought or developed.
  7. Motivates and supports family involvement and the identification and inclusion of informal supports.
  8. Complete all CHCS and contract specific required documents in timely manner and to standard
  9. Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned
  10. Comply with and participate in Quality Assurance and Quality Improvement processes
  11. Adhere to established CHCS Employee Handbook and policies and procedures
  12. Other duties as assigned



  1. Bachelor's degree from an accredited university or college in a Behavioral Health field
  2. Minimum One (1) year experience working with children with serious emotional disturbance (SED)
  3. Must be able to successfully complete the Qualified Mental Health Professional Community Services (QMHP-CS) within six (6) weeks of hire or transfer
  4. Proficiency with standard Microsoft Office applications and system databases  
  5. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  6. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business 
  7. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  8. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care.
  9. Must be able to transport consumers occasionally.


  1. Current Wraparound Training  
  2. Prior working experience with or knowledge of trauma-informed care
  3. Bilingual (English/Spanish)

Compensation Package

Salary Range $37,294.00 - $55,941.00 Per Year
