The Care Manager (Adult Services) works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to adults in a person-centered trauma informed, individualized focused approach utilizing evidenced-based practices delivered to fidelity. These services are delivered in multiple types of settings including field-based services in the individual’s home or other identified locations as well as clinic-based services. Individuals served may have multi-system involvement such is Criminal Justice, Child and Adult Welfare, Veterans, Homelessness, etc.


The Hospital Liaison works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to adults in a person-centered trauma informed, and individualized focus approach. These services are delivered in hospital settings with patients in the emergency room, and inpatient psychiatric units. To conduct any CHCS intake function to include MCOT, CMOT, Adult Behavioral Health and Child Behavioral Health, as well as expedited screenings for CHCS private psychiatric bed and state hospital bed access.


Adult Services – Outpatient – This position works in the Outpatient programs and among the various Adult facility locations. Services in this program include individual and group outpatient services for adults which includes Crisis Services, Medication Management, Psychosocial Rehabilitation, Case Management, Skills Training and Complex Services. Other programs include Substance Use Disorder and Criminal Justice.

Adult Crisis Services – This position works in the Adult Crisis Unit or the Josephine Recovery Center.   Services in the unit include emergency and planned crisis care for adults ages 18 and over who have a behavioral health diagnosis or need. The program provides meals, medication monitoring, supervised recreation, skills training, community referrals, and 24-hour supervision in a secure environment. The Josephine Recovery Center is not a secure environment, but provides the same services as The Adult Crisis Unit. In addition to these sites CHCS operates a Mobile Crisis Outreach Team (MCOT), a resource designed to bring a crisis worker into the community to provide a face-to-face assessment and intervention, follow-up and relapse prevention services.

CHCS MEDCOM – Serve as the primary role to provide aggressive referral, linkage, and transition into the next level of care, in partnership with PES providers and CHCS MEDCOM MCOT clinicians. Identify patients currently being treated by CHCS and provide connection back to assign CHCS Care Manager as appropriate. Identify individuals appropriate for the CHCS Complex Care Division (Josephine Recovery Center, Integrated Treatment team, Detox Unit, etc.) and facilitate these referrals to the programs as needed; Facilitate expedited follow-up appointments with CHCS or the appropriate outpatient provider; monitor and track progress of compliance with those appointments. Identify patients who have been non-compliant in follow-up appointments, present details for case reviews with STRAC, CHCS Clinical Administrator, and CHCS MEDDCOM, MCOT Clinician to determine if home visits are necessary.



  1. Complete and maintain required training, credentialing, licenses, and background checks
  2. Regularly report to work on days and times of approved schedule in order to accommodate client and business needs
  3. Participate in centralized scheduling process
  4. Provide direct individualized clinical services 60% of hours worked that may include:Complete all CHCS and contract specific required documents in timely manner and to standard
    • Assessment
    • Recovery Planning
    • Psychosocial Rehab Services (Individual and/or Group)
    • Skills Training and Development (Individual and/or Group)
    • Medication Training and Support (Individual and/or Group)
    • Routine Case Management
    • Psychoeducation
    • Case Coordination 
    • Community referrals
    • Crisis Services
    • Other clinical interventions as assigned
    • Case Transfers
    • Discharge Planning
  5. Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned
  6. Comply with and participate in Quality Assurance and Quality Improvement processes
  7. Coordinate coverage for planned absences in advance with supervisor
  8. Adhere to established CHCS Employee Handbook and policies and procedures
  9. Other job duties as assigned


  1. Bachelor's degree from an accredited university or college in a Behavioral Health field
  2. Must be able to successfully complete the Qualified Mental Health Professional Community Services (QMHP-CS) within six (6) weeks of hire or transfer
  3. Proficiency with standard Microsoft Office applications and system databases
  4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  5. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business
  6. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  7. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care.
  8. Must be able to transport consumers occasionally.
  1. One (1) year experience providing similar services
  2. Prior working experience with or knowledge of trauma-informed services
  3. Bilingual (English/Spanish)
