YES Waiver Wraparound Supervisor

Salary Range: $41,769.00 - $62,654.00 Per Year
Work Schedule: Monday - Friday 8:30 AM 5:30 PM


The YES Waiver Wraparound Supervisor will work closely with the Clinical Director by establishing and implementing program process and procedures that will enhance wraparound facilitatorsí skills and capabilities in support of the National Wraparound Initiative (NWI) programís goals and objectives.


  1. Provide effective leadership to a team of six to eight (6-8) Wrap-Around facilitators by working with or establishing program goals to include creating actionable case management plans that are incorporated within measurable standards that meet key program and process requirements
  2. As supervisor, establish direct report performance requirements and expectations and provide timely feedback
  3. Plan daily work activities and assign tasks based on skill level, knowledge, and experience
  4. Responsible for training personnel on tasks required for program success and career development
  5. Organize work flow to ensure efficiency of applied resources
  6. Coordinate diverse activities through communication with staff, key personnel to include internal and external agencies
  7. Develop and incorporate key success strategies for facilitators by providing crucial coaching guidance along with traveling with facilitators to various case management appointment
  8. Provide timely feedback on techniques and methods used to ensure facilitators are properly trained and knowledgably equipped to champion programís success                                        
  9. Routinely review Quality Assurance evaluation criteria to ensure compliance with CHCSís expectations for Quality Assurance to include applicable governing directives                                                  
  10. Assist Clinical Director in reviewing wraparound goals, objectives, and plans while providing feedback as needed to make necessary adjustments in meeting and maintaining program goals, standards, and objectives
  11. Other duties specified which correlate with program management and implementation to include: managing a small caseload as needed and/or providing temporary backup/support  
  12. Evenings and weekends are required to best suit the schedules of the children and families served


  1. Bachelorís degree in social science/behavioral health
  2. Three (3) yearsí of related experience
  3. Intermediate skill in electronic calendar, computer software including Microsoft Word, Excel, PowerPoint, system database, internet/intranet
  4. Strong verbal and written communication
  5. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  6. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business
  7. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  8. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.


  1. Master degree in Behavioral Science, Counseling, or Social Work
  2. Bilingual/Spanish speaking, read, and write
  3. One (1) year as a wrap facilitator

Compensation Package 

Salary Range $41,769.00 Ė $62,654.00 Per Year
