Direct Care Specialist - Part-time

PURPOSE  

The Direct Care Specialist will provide routine care for consumers, and complete associated documentation for the consumer and program 

TASKS


  1. Provide personal care services (assistance with activities of daily living and independent living skills) 
  2. Assist the activities supervisor with recreational activities (n/a for some programs) 
  3. Provide protective supervision (observation and monitoring)
  4. Assists with Ambulation and mobility (Assistance may range from total physical assistance with or without adaptive equipment, to verbal support) 
  5. Ensures clinical documentation is submitted in accordance with Center standards and meets all designated requirements 
  6. Assist with consumer assessments, and with development of goals and objectives 
  7. Other duties as assigned

MINIMUM REQUIREMENTS 

  1. High School Diploma or GED or Competency test with three personal reference letters
  2. Six (6) months’ of experience in human service care 
  3. Experience with operating telephone, fax machine, copy machine, calculator, motor vehicles; (lifting devices and other physical therapy equipment may be required for some programs)
  4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  5. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business 
  6. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  7. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.
