Clinical Practitioner II - CBH

POSITION SUMMARY: The Childrenís Behavioral Health (CBH) Clinical Practitioner II works within a multi-disciplinary team to provide an array of direct (face-to-face) and indirect clinical services to children/adolescents as well as caregivers in a child-centered, family focused approach utilizing evidenced based practices delivered to fidelity. These services are delivered in multiple types of settings including field based services in the childís home, the childís school, or other identified community locations as well as clinic-based services provided from multiple CBH clinic locations in San Antonio.

UNIT/PROGRAM UNIQUE REQUIREMENTS OR DESCRIPTION: This position works in the CBH System of Care SAMHSA Unit 9254 (Non-TRR Unit) and rotates among the Story Lane or community organization location and the clientís home, daycare, or school. The unit is funded through a 4 Year Federal Grant from the Substance Abuse and Mental Health Services Association awarded to the City of San Antonio. CHCS is the lead agency for the grant and braids funding with two other System of Care Units in the CBH Division. Services in the unit include Skills Training and Development, Caregiver Training and Support, Case Management, Group Activities, and Case Coordination. Children served in this unit are ages 3-8 years of age who have an elevated score on the Pediatric Symptoms Checklist (PSC). This unit provides service delivery to 250 unduplicated children each year and also works with a Governance Council to develop community infrastructure around mental health needs of children with an emphasis on the development of a Community Parent Network. The unit consists of a Project Director, a Clinical Administrator, Clinical Practitioners, Business and Support Personnel and access to Certified Family Partners and Medical Services. Clinical Practitioners must be available for early morning, early evening, and weekend appointments to accommodate family needs. Participation in community events, meetings, and trainings are required of all employees in this unit.

ESSENTIAL JOB FUNCTIONS:

  1. Complete and maintain required training, credentialing, licenses, and background checks
  2. Regularly report to work on days and times of approved schedule that includes some early morning, early evening, and weekends to accommodate client and business needs
  3. Participate in centralized scheduling process
  4. Provide direct individualized clinical services 60% of hours worked that may include:
    1. Assessment
    2. Recovery Planning
    3. Psychosocial Rehab Services (Individual and/or Group)
    4. Skills Training and Development (Individual and/or Group)
    5. Medication Training and Support (Individual and/or Group)
    6. Routine Case Management
    7. Psychoeducation
    8. Case Coordination
    9. Community referrals
    10. Family Training (Individual and/or Group)
    11. Parent Support Group
    12. Family Case Management
    13. Crisis Services
    14. Transition Age Youth Services
    15. Other clinical interventions as assigned
    16. Case Transfers
    17. Discharge Planning

 

  1. Complete all CHCS and contract specific required documents in timely manner and to standard
  2. Ensure that all required SAMHSA evaluation assessments are done within the appropriate timelines
  3. Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned
  4. Comply with and participate in Quality Assurance and Quality Improvement processes
  5. Coordinate coverage for planned absences in advance with supervisor
  6. Adhere to established CHCS Employee Handbook and policies and procedures
  7. Other job duties as assigned

 

MINIMUM REQUIREMENTS:

  1. Masterís degree in behavioral health field.
  2. Must be able to be credentialed with all Center payers    
  3. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  4. Proficiency with standard Microsoft Office applications and system databases
  5. Must be able to pass required background check(s).
  6. Must pass random drug screening(s).
  7. Must carry business phone and respond to clients and supervisor in timely manner.
  8. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  9. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and      supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  10. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and      horizontal), using fingers, hands, feet, legs and torso in various care.
  11. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50      pounds.
  12. The employee must be able to occasionally transfer a consumer.

Experience: The ideal candidate will have experience delivering culturally competent mental health services to children/adolescents preferably in a community mental health clinical setting. The candidate has a strong work ethic and positive attitude. The candidate has experience managing multiple caseload assignments and can demonstrate clinical understanding of assessment, treatment planning, engagement, crisis intervention, and discharge planning.
