Provider Monitor

Department: 1401 - Respite
Location: San Antonio, TX
Work Schedule: Mon - Fri: 8:00am - 5:00pm

The Provider Monitor Lead will provide and coordinate direct services for consumers, and complete associated documentation for the consumer and program, providing quality control oversight through guidance and coordination to Provider Monitors.

TASKS

  1. Provide, coordinate and monitor medical, social, educational, and/or other appropriate services, to include foster care services, as needed for individuals on assigned caseload
  2. Ensure clinical documentation is submitted in accordance with Center standards, and meets all designated requirements                                                              
  3. Complete Implementation Plan annually and resolve all discrepancies; annually obtain and provide consent forms authorizing services
  4. Comply with and participate in Quality Improvement processes
  5. Link and refer consumers to appropriate services (internal and/or external) as required.
  6. Other duties as assigned

MINIMUM REQUIREMENTS 

  1. Bachelor's degree in a Human Services field (Sociology, Psychology, Family Studies, etc.) from an accredited 4 year college or university
  2. 1 - 2 years of full-time experience with individuals with developmental disabilities
  3. Proficiency with standard Microsoft Office software and system databases
  4. Proficiency operating telephone, fax machine, copy machine, calculator, and motor vehicles       
  5. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  6. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  7. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  8. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching(vertical and horizontal), using fingers, hands, feet, legs and torso in various care.  The employee must be able to regularly lift and/or move up to 40 lbs and occasionally must lift and/or move up to 50 lbs.  The employee must be able to occasionally transfer a consumer.
