Medical Records Specialist II

Department: 4424 - FACT
Location: San Antonio, TX
Work Schedule: Monday through Friday 8:30 AM to 5:30 PM

PURPOSE

The Medical Records Specialist II is responsible for creating, maintaining, and focused reviewing of client medical records, reports and information inquiries for the program.


TASKS

  1. Creates, assembles, pulls, refiles and maintains client charts 
  2. Performs focused reviews of charts, ensuring assigned charts are uniform, ,orderly, complete, and accessible to authorized individuals 
  3. Maintain the Unit's New Generation Medication caseload 
  4. Process release of information requests in accordance with HIPAA and Center guidelines 
  5. Serves as backup for other Business Support Services positions 
  6. Other duties as assigned

 MINIMUM REQUIREMENTS 

  1. High School Diploma or GED 
  2. Three (3) years’ medical records experience (may substitute an approved training program for experience requirement) 
  3. Proficiency with standard Microsoft Office applications and system databases       
  4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  5. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business 
  6. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.

  PREFERRED 

       1. Bilingual (English / Spanish)
