Activities Specialist

Department: 1555 Day Activity Health Services (DAHS)
Location: San Antonio, TX
Work Schedule: Monday through Friday 8:30 AM to 5:30 PM

PURPOSE  

The Activities Director will provide routine care for consumers, and complete associated documentation for the consumer and program 

TASKS

  1. Provide personal care services (assistance with activities of daily living and independent living skills) 
  2. Planning and scheduling activities and recreational activities for the unit  
  3. Complete all paper work for the nutrition food program  
  4. Data entry  
  5. Provide protective supervision (observation and monitoring) 
  6. Assists with Ambulation and mobility (Assistance may range from total physical assistance with or without adaptive equipment, to verbal support) 
  7. Ensures clinical documentation is submitted in accordance with The Center For Health Care Servicesís standards and meets all designated requirements 
  8. Other duties as assigned  

MINIMUM REQUIREMENTS 

  1. High School Diploma or GED
  2. A Bachelorís degree from an accredited college or university, and one year of full-time experience working with elderly people or people with disabilities in a human service or medically related program Ö. OR Sixty (60) semester hours from an accredited college or university, and two (2) years of full-time experience working with elderly people or people with disabilities in a human service or medically related programÖ. OR Completion of an activities directorís course, and two (2) years of full-time experience working with elderly people or people with disabilities in a human service or medically related program
  3. Experience with operating telephone, fax machine, copy machine, calculator, motor vehicles; (lifting devices and other physical therapy equipment)
  4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  5. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  6. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  7. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), and using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.

PREFERRED

       1. Bilingual (English / Spanish)
