Location: San Antonio, TX
Work Schedule: Monday through Friday 8:30 AM to 5:30 PM

PURPOSE 

The LVN Nurse Case Manager screens patients to determine unmet needs, identifies knowledge gaps concerning disease. Management and treatments, coordinates appropriate resources or services required to meet an individualís health needs. Promotes quality cost effective outcomes with the goal of improved care coordination and increased involvement of the Individual/family/caregiver in the decision making process to reduce hospitalizations, re-admissions and ER visits. May assist with laboratory, EKGs and other medical treatments, maintain pharmaceutical oversight of medication samples and complete associated documentation.


TASKS

  1. Provide comprehensive assessment/case management and education including self-management support, psychosocial education on Axis I-III diagnoses, symptom management, health promotion, connection/referral to appropriate physical or substance abuse providers as well as community based organization social supports to decrease the barriers preventing the patient from attending appointments and following the authorization recovery plan.
  2. Facilitate care coordination with specialty medical care as well as acute and outpatient medical and substance abuse services and other care managers involved in supporting the individual.
  3. Identify and/or work with others to identify patients with high risk of adverse health outcomes (e.g., death, disability, in-patient admission, ER visits).
  4. Conduct assessment of patient condition, needs, clinical and psychosocial barriers.
  5. Provide health care instructions to the patient as appropriate per assessment and plan.
  6. Complete vital signs and ROS.
  7. Communicates with the prescriber regarding the patientís needs and reports changes in the patientís condition; obtains/receives prescriber orders as required.
  8. Develop and implement the patient centered recovery plan in collaboration with the patient.
  9. Provide community/home-based services and telephonic support.
  10. Identify and address patient risk factors and/or obstacles to care.
  11. Maintain and document in patient EMR in accordance with CHCS policy.
  12. Other duties as assigned

 MINIMUM REQUIREMENTS 

  1. Bachelor degree in behavioral sciences with ability to obtain QMHP-CS credential.
  2. Current Texas LVN licensure
  3. Knows and conforms to the Texas Nurse I Practice Act and the Boardís rules and regulations as well as all Federal, State, and local laws, rules, or regulations affecting the LVNís current area of nursing practice
  4. Three (3) to five (5) years of experience as a LVN/Case Manager providing field based psychosocial rehabilitation and case management services.
  5. Demonstrated ability to work effectively with others to coordinate patient access to care and community resources.
  6. Experience working with SMI population.
  7. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  8. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  9. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  10. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.
