Community Outreach Specialist - Part-Time

Work Schedule: Part-time Flexible day/hours

 

PURPOSE 

 

The Community Outreach Specialist will provide routine direct services and linkages/referrals for consumers, and complete associated documentation for the consumer and program.

 

TASKS 

 

  1. Engage in traditional (venue-based) and non-traditional (online) outreach activities for the purpose of providing HIV/STI related: health information, referrals and access to services, and recruitment for testing
  2. Approach and enthusiastically interact with community members during venue-based outreach activities
  3. Engage clients, community partners, and all persons using a non-judgemental and compassionate approach. Authentically practice kindness.
  4. Provide referrals to multiple community programs
  5. Recruit individuals into additional program services, such as Sexual Health Curriculum, focus groups, adapted social networking strategy
  6. Identify and share potential outreach venues or ideas to reach population(s) of focus
  7. Obtain clientsí sexual and substance history and systematically record the data on the appropriate form(s) 
  8. Provide testing for HIV and Hepatitis C and deliver test results in accordance with programmatic and state standards
  9. Conduct results counseling for all clients with positive test results and link all clients, with positive HIV test results, to HIV early intervention services (HEI); refer all clients with Hepatitis C positive test results to treatment
  10. Manage a caseload of clients to include Sexual Health Advocates, volunteers, and call back scheduling
  11. Enter test records in applicable databases within 23 hours in accordance with CHCS policies 
  12. Participate in programmatic quality assurance procedures
  13. Complete all training in a timely manner
  14. Create harm reduction packs for community members
  15. Other responsibilities as based on programmatic need

 

MINIMUM REQUIREMENTS 

 

  1. High School Diploma or GED 
  2. Willingness to competently engage with population of focus
  3. Proficiency with standard Microsoft Office applications and system databases
  4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  5. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  6. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  7. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.
