Clinical Practitioner II

Department: 9251 - Crisis Services - Children
Location: San Antonio, TX
Work Schedule: Varies and includes on-call


The Clinical Practitioner II will provide direct services for consumers, and complete associated documentation for the consumer and program.


  1. Provide direct clinical services, assessments and develop treatment plan for consumers
  2. Ensure clinical documentation is submitted in accordance with Center standards, and meets all designated requirements
  3. Link and refer consumers presenting to clinic to appropriate services (internal and/or external) as indicated
  4. Comply with and participate in Quality Assurance and Quality Improvement processes
  5. Serves as the Lead Case Manager in the assigned unit or Program as well as back up to the Clinic Administrator
  6. Other duties as assigned


  1. Master's degree in Behavioral Science or related field OR for FACT/ACT/MCOT/CRISIS, Bachelorís Degree in Behavioral Science or Related Field AND 6 months full-time experience in an ACT/FACT/MCOT/CRISIS Program working with adults, OR for Permanency Project, Bachelorís Degree in Behavioral Science or Related Field AND Current Texas License as a Licensed Chemical Dependency Counselor (LCDC)
  2. For EIS, 6 months full-time experience in an Early Childhood Intervention Program working with children ages 0-12 or bachelorís degree in social work, licensed by Texas State Board of Social Work Examiners and 6 months full time experience
  3. Two yearsí experience in teaching, conducting/facilitating training for groups (Except for FACT/ACT/MCOT)      
  4. Must be able to be credentialed with all Center
  5. Proficiency with standard Microsoft Office applications and system databases                
  6. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  7. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  8. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
