Primary Care Provider FNP/PA

Department: 1144 - Paul Elizondo Primary Care
Location: San Antonio, TX
Work Schedule: Monday through Friday 8:30 AM to 5:30 PM


The Primary Care Provider provides direct high quality and evidence based patient medical care including diagnosis, treatment and/or proper referral, follow-up, education and counseling. Duties may include seeing children and adolescents. PCP will participate in quality assurance, quality improvement, peer review and special project.  


  1.  Manages and maintains a program of comprehensive health for patients to include preventative medicine, behavioral sciences, and community health. 
  2. Examine patients, formulates diagnostic plans, defines and orders required diagnostic testing.  
  3. Interperts examination finding and test results and implements treatment plans. 
  4. Conducts medical examinations and reviews collateral information of assigned patients in accordance with applicable guidelines.  
  5. Timely completes required EMR and all other documentation according to medical license and Center standards.        
  6. Evaluates urgent conditions of patients in a competent and timely manner.
  7. Responsible for coordinating medical care with internal and external medical specialties as needed.  
  8. Determines needs for consultation and assists in coordination of medical care and treatment provided at the direction of other specialists.
  9. Prescribes and assesses effectiveness of pharmaceuticals, other medications and treatment regimens as appropriate for patient medical accordance with the Centerís Prescribing Practices Policies and Procedures 
  10. Ability to supervise, advise, train clinical professionals, residents, and/ or students in area of health-related topics.  
  11. Orders labs in accordance with appropriate guidelines.
  12. Other duties as assigned


  1.  Current Texas Physician Assistant OR RN license, Advanced Practice license from the Texas Board of Nurse Examiners, Texas Department of Public Safety Controlled Substance Registration Certificate (DPS), and U.S. Drug Enforcement Agency Substance Registration Certificate (DEA)
  2. Completion of an accredited relevant Residency Program if applicable (Family Nurse Residency)
  3. Must apply to be credentialed with all Center contracted payors
  4. Must have working knowledge of the principles of the development, implementation, and documentation of individualized care and treatment
  5. Must have working knowledge of State and Federal laws and professional standards related to medical treatment
  6. Must have ability to evaluate patients, set up and carry out an effective course of treatment utilizing the expertise of other clinical personnel with a minimum of direction
  7. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  8. Must be proficient with standard Microsoft Office applications and EMR systems
  9. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business 
  10. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  11. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.


  1.  Experience practice in a center serving diverse under-insured and under-served populations.
  2. Additional language capability strongly desired.
  3. Bilingual (English/Spanish)
