Behavioral Health Consultant

Work Schedule: M-F: 8:30 a.m. - 5:30 p.m.

PURPOSE 

The Behavioral Health Consultant (BHC) is an embedded full-time member of the primary care team and provides clinical behavioral health services within a primary care setting, including cognitive and behavioral interventions to individuals with mental health or substance use disorders. He or she collaborates with primary care providers to enhance understanding of patients, coordinate care, plan and implement treatments, and monitor progress. The primary role of a BHC (at CHCS) is to provide consultation and support to the Primary Care Provider (PCP) before, during and after patient visit. Interdisciplinary decision making about medical treatment plan often occurs as the primary care provider looks to the BHC to understand the influence of psychiatric conditions on health conditions.

TASKS

  1. The Behavioral Health Consultant (BHC) maintains an open access schedule and is proactive in providing behavioral health consultations to new and returning primary care patients.
  2. The BHC is grounded in the science of the psychological principles of behavior change, and demonstrates knowledge of behavioral medicine chronic health conditions, and is familiar with primary care practice.
  3. The BHC quickly assesses and synthesizes medical and psychiatric data to formulate effective and evidence-based clinical recommendations.
  4. The BHC should demonstrate a comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.
  5. The BHC is actively involved in treatment plan changes, discussion of medication regimens, and provision of cognitive- behavioral interventions in particular for patients who are not improving as expected
  6. The BHC provides brief, targeted, real-time assessment/interventions to address a wide range of co-morbid health, mental health, and substance abuse problems that uniquely arise in a behavioral health clinical population during primary care visits.
  7. The BHC collaborates with case managers, nurse care coordinator, and integrated care coordinator to address the psychosocial aspects of chronic and acute diseases.
  8. The BHC partners with patients in a treatment approach that builds resiliency and encourages personal responsibility for health with an emphasis on self-management and functional outcomes. Number of consultation sessions is limited (a total of four (6) sessions on average) with each session lasting fifteen (15) to thirty (30) minutes.
  9. The BHC connects patients with peer supports, community resources, and crisis assessment and/or intervention as necessary.
  10. The BHC promotes the importance of social supports and good lifestyle choices.
  11. The BHC takes an overall view of the patient in context: integrating multi-disciplinary information into a working conceptualization of the case, sharing with the team, which enhances care and drives treatment.
  12. The BHC also uses tools (i.e. patient registry) to identify and interpret population health outcome data for timely recommendations to the primary care team for individual patient care. Treatment by the PCP and BHC is documented in a shared treatment plan.
  13. The BHC actively participates in formal (i.e. team huddles, medical provider meetings) and informal (i.e. unscheduled consultations) communications in particular with the primary care team. 
  14. The BHC is a scientist-practitioner who consistently and frequently references scientific theory and recent research findings in health psychology and behavioral medicine to develop, implement and formalize BHC-specific clinical protocols and pathways in a primary care setting.
  15. Other duties as assigned

MINIMUM REQUIREMENTS

  1. Masterís degree in Social Work, Counseling, Psychology, Marriage and Family Therapy or related field; two years providing counseling and competency in providing cognitive behavioral interventions required.
  2. Psychologist, a doctoral degree (PhD or PsyD) in clinical or health psychology from an APA accredited program; APA-accredited internship/residency preferred.
  3. Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Counselor (LMFT) or Licensed Psychologist required.
  4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.
  5. Evidence of being flexible, high-energy, and team-oriented with excellent interpersonal and communication skills.
  6. Must possess clinical assessment skills to formulate effective and evidence-based clinical recommendations.
  7. Demonstrated ability to deliver a broad range and mix of brief cognitive and/or behaviorally-based interventions to activate patient self-management of their whole health.
  8. Proficient in Microsoft Office Suite: Outlook, Word, Excel, Power point, etc. Ability to efficiently document in electronic health record.
  9. Must maintain a valid driverís license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Companyís auto insurance carrier if you drive your vehicle during company business.
  10. Ability to drive and have access to a car, maintain. Frequent in-town periodic regional travel required.
  11. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
  12. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.
PREFERRED

1. Specialized training in health psychology; experience working in primary care settings.

2. Demonstrated intermediate to advanced competency in deploying evidence-based practices commonly used in PC settings, i.e. Motivational Interviewing (MI), CBT, Problem Solving Therapy (PST), Behavioral Activation (BA), Relapse Prevention, Acceptance and Commitment Therapy (ACT), biofeedback, etc


3. For a Psychologist, experience providing Behavioral Health Consultant services in a primary care setting preferred.

4. Experience with psychological assessment and treatment of children and adolescents.

5. Bilingual in both English and Spanish.
