LVN – Primary Care- Full Time (Days)

Department: 1143 - 1115 ABH High Utilizer Expansion
Location: San Antonio, TX
Work Schedule: Monday through Friday 8:30 AM to 5:30 PM

PURPOSE 

The LVN – Primary Care will provide patient care coordination and direct nursing services for patients, and assist with laboratory, EKGs and other medical treatments, maintain pharmaceutical oversight of medication samples, and complete associated documentation for the patient and program.

TASKS

1. Provide professional nursing care to individuals by performing nursing assessments regarding the health status of the client, and review of all OTC medication being taken and history of substance and/or alcohol abuse.

2. Adhere to the nursing diagnosis which serves as the basis for care, develop and implement plan of care based on nursing assessment and diagnosis, and evaluate consumer's responses to nursing interventions.

3. Assist with laboratory collection, EKGs, and other medical treatments/interventions appropriately, taking initial health history on new patients.

 4. Maintains pharmaceutical oversight of medication samples for specified units participating in the program through assigned unit level staff, maintaining accurate account of stock medications, controlled substances, and consumer medications.

5. Coordinate medication distribution with the Center's pharmacy contract, and coordinates services with pharmacy representatives and/or companies.

6. Provide Comprehensive care management including self-management support, health promotion, connection/referral to appropriate physical, mental health or substance abuse providers as well as community based organization social supports to decrease the barriers preventing the patient from attending appointments an following the plan of care.

7. Facilitate care coordination with specialty medical care as well as acute and outpatient medical, mental health, substance abuse services and other care managers involved in supporting the individual.

8. Identify and/or work with others to identify patients with high risk of adverse health outcomes (e.g. death, disability, in-patient admission, skilled nursing facility (SNF) admission or ED visit).

9. Conduct assessment of patient condition, needs, preferences, clinical and psycholsocial barriers.

10. Implement the patient approved plan of care in collaboration with the patient through the practice’s care team, community – home-based visits and telephonic support.

11. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.

12. Other duties as assigned

MINIMUM REQUIREMENTS 

1. Current Texas LVN licensure and minimum vocational education component.


2. Demonstrated ability to work effectively with others to coordinate patient and access care to support services by providing input related to changes that may enhance the practice effectiveness to include; patient progress and barriers encountered.

 
3. Experience planning pre-visit and post-visit follow-up care, as well as preparing for an participating in case review meetings to share discoveries, concerns and collaborate in the development plans for care.


4. Three (3) years’ experience as an LVN in a primary care setting.


5. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies.

6. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business.

7. Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.

8. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. The employee must be able to occasionally transfer a consumer.

PREFERRED
1. One (1) year of substance abuse and/or psychiatric nursing experience
